HMIS Policies and Procedures

The NYC HMIS Policies and Procedures have been revised and the Data Management Committee will be making a motion at the May Steering Committee meeting to adopt the revised Policies and Procedures.
As per HUD's requirements, each CHO must be trained on the NYC HMIS Policies & Procedures, especially as they relate to security, privacy, and data collection. The revised document, main changes, and training dates can be found on the hmis page. Click here to view.


Evaluation Process Assistance

DHS will be hosting "office hours" to assist providers with questions on how to complete the portfolio documents on Monday, April 8 from 2:00pm - 5:00pm and Friday, April 12 from 2:00pm - 4:00pm in room 1602 at 33 Beaver St. Please contact Aleida Valentin (avalenti@dhs.nyc.gov, 212-232-0529) if you would like to sign up for one of the sessions, and come prepared with the evaluation instructions, portfolio documents, and APR(s). These sessions are optional.



NYC CCoC List

In order to facilitate and assist providers in addressing NYC CCoC requirements, policies and procedures such as the NOFA and Evaluation processes, going forward DHS has divided the provider list between Merih Anil and Aleida Valentin as follows:


Merih Anil (manil@dhs.nyc.gov 212-232-0830) will work with agencies beginning with Addicts Rehabilitation Center Fund through Kenmore Housing Development Fund (A-K).


Aleida Valentin (avalenti@dhs.nyc.gov 212-232-0529) will work with agencies beginning with Lenox Hill through the YMCA of Greater New York (L-Y).

2013 Evaluation Process

The NYC CCoC has announced the release of the 2013 Evaluation Process Overview and Instructions and 2013 Evaluation Tool documents. They are now available on the NYC Coalition on the Continuum of Care website and can be found on the the Evaluation Process page.

2012 NOFA Tier 1 Renewals Announced

 

Below please find a link to HUD's announcement of Tier 1 renewals, and a link to a list of Tier 1 renewals for New York State:


Hurricane Sandy

 

Please see the attached guidance (Disaster Eligibility Memorandum) to assist you in assessing the eligibility of participants for homeless assistance who have been affected by Hurricane Sandy. The declared counties for individual assistance and public assistance are now: Bronx County, Kings County, Nassau County, New York County, Orange County, Putnam County, Queens County, Richmond County, Rockland County, Suffolk County, Sullivan County, Ulster County and Westchester County. Please distribute this information to providers operating in the declared counties to ensure that persons or families affected by the storm meet applicable criteria before entry into HUD assisted homeless housing.

HEARTH Act Implementation Updates

 The Homeless Emergency Assistance and Rapid Transition to Housing Act of 2009 (HEARTH Act), enacted into law on May 20, 2009, consolidates HUD’s competitive homelessness assistance grant programs and names this merger, the Continuum of Care (CoC) Program.

  • HUD posted the interim regulation for the new Continuum of Care Program to the HUD Homelessness Resource Exchange website at www.hudhre.info on Saturday, July 14. HUD expects the regulation to be published in the Federal Register in the coming week. The interim regulation will be effective 30 days after publication in the Federal Register.

  • CoC Program recipients and subrecipients are reminded that the final Homeless Definition is in effect for administration of the CoC Program interim rule. The final Homeless Definition is posted on the HUD HRE.

  • To assist CoCs, stakeholders, recipients and subrecipients in the successful implementation of the CoC Program interim rule, HUD is conducting a phased release of guidance documents, tools, and webcasts. Following are the dates of release of introductory user guides and tools to the HUD HRE: 

    • Week of July 16

      • Introductory Guide to the CoC Program User Guide – provides an introduction to the CoC Program and the requirements in the interim rule.

      • Hyperlinked version of the CoC Program interim rule

      • Week of July 23

        • Crosswalk of Changes for Program Eligible Costs: Continuum of Care Program, Supportive Housing Program (SHP), and Shelter Plus Care (S+C) Program Regulations – provides a quick reference guide to grant related citations of the CoC Program interim rule eligible costs. The table also provides citations for relevant sections of the prior grant regulations (SHP and S+C) to facilitate identifying and comparing regulatory changes that will affect CoC grant recipients and subrecipients.

        • Leasing and Rental Assistance Transition Tool – provides CoC program recipients assistance with transitioning former SHP and S+C projects to rental assistance and leasing projects.

        • Early August

          • CoC Program Funding for Homeless Management Information System (HMIS) User Guide – provides an overview of the HMIS and the CoC’s responsibilities in relation to HMIS and describes how CoC Program funds can be used by the HMIS Lead and by other recipients to support the implementation of a HMIS.

          • CoC Program Components and Eligible Costs User Guide – provides CoCs, recipients, and subrecipients an overview of the five CoC components and costs that are eligible and ineligible for each component.

          • Continuum of Care Program: Implementing the CoC and the FY 2012 CoC Registration Process

          Questions:

          • HUD has created a new section of the HUD Virtual Help Desk, at www.hudhre.info, for questions about the new regulation. The CoC Program Policy Help Desk will be available on Monday, July 16, 2012. HUD has taken great care to anticipate questions and include as much detail in the regulation as possible. Therefore, please ensure that you read the regulation in its entirety before submitting a question. This will decrease the number of duplicative questions and responses, and increase the response time for questions that were not answered in the regulation.

          • HUD is also developing a comprehensive library of guidance documents to assist recipients and subrecipients in administering the CoC Program and the new definition of homelessness. HUD appreciates your patience as these documents are being completed. Many of your questions will be answered as the documents are published. Additional listservs will be forthcoming announcing the availability of the guidance documents and tools.


New APR for CoC Grants Live in e-snaps

The new CoC Annual Performance Report (APR) is live in e-snaps for Supportive Housing Program (SHP), Shelter Plus Care (S+C) and Section 8 Mod Rehab grants expiring June 1, 2011 or later. The new CoC APR is available under the e-snaps Funding Opportunity: CoC Full Annual Performance Report.  A CoC APR Guidebook will be posted on HUD HRE (www.hudhre.info) by Friday, June 17th.  The Guidebook will walk grantees through the submission process for the CoC Full APR. 

SHP, S+C and SRO grants that expire between June 1, 2010 and May 31, 2011 must submit a Transition APR (TAPR) under the e-snaps Funding Opportunity:  CoC Annual Performance Report
