Shelter Plus Care (S+C) New Project Documentation Requirements

Submit the items in the order listed for each section by Friday, May 25th, 2007 by NOON. Please paperclip each section together, DO NOT staple or bind. Please submit only the original of all the documents stated below - no copies are needed.

Section 1: Applicant Documentation (One per applicant)
SF 424 Application for Federal Assistance
  List of all the applicant projects in priority number order with project name and requested amount (includes both SHP and S+C projects) (DHS will provide after submission of application).
Documentation of Applicant Eligibility
SF- LLL Disclosure of Lobbying Activities (only complete if applicable)
Applicant Code of Conduct *  
HUD 40090–4 Applicant Certifications
   
Section 2: Project Documentation (Complete one per project of each applicant)

HUD 96010 Logic Model
HUD 2880  Applicant/Recipient Disclosure Update Report
HUD 2991 Certification of Consistency with Con Plan (DHS will provide at training on the 11th)
SF 424 Supp  Survey on Ensuring Equal Opportunity (Optional)
   
* Applicant Code of Conduct Requirements
If you submitted an application to HUD during FY2005 that included a code of conduct, and it has been identified by HUD on its Web site at http://www.hud.gov/grants/index.cfm  and if that information is still valid, you will not have to resubmit another copy.   To see if your Code of Conduct is available please go to www.nychomeless.com to review HUD's current list on their website.
However, if your code of conduct is not listed as received on the HUD Web site, or if the information you submitted has changed (e.g., the person who submitted the previous application is no longer your authorized organization representative, the organization has changed its legal name or merged with another organization, or the address of the organization has changed, etc.), you must submit your current code of conduct with your 2007 application for assistance.
